This form is to be used by retailers requesting the churn of a customer from one retailer to another by Power and Water Corporation.
Power and Water requires verified customer consent before carrying out any Historical Data Requests on behalf of a customer’s retailer or authorised representative. A completed Customer Consent Form must accompany all such requests. Any request submitted without a signed consent form may be declined.
